What to expect at your new patient appointment:

When you arrive at your new patient appointment, we will direct you to our consult room to complete your paperwork through our Quick Check-in process.

During the Quick Check-in process you will go through the following:

  • Medical History
  • HIPPA Forms

Please bring your insurance card to your new appointment and plan to spend about two hours during your new patient appointment. Depending on previous dental history and patient’s age, new patient appointments can vary in time. During this appoitment Dr. Krivarchka will review your past medical and dental history. He will do a comprehensive exam, take necessary digital X-rays, intraoral photos, and discuss findings and treatment needed with patients. You will then be seen by our dental hygienist to be evaluated for your dental cleaning and patient education. Once your new patient appointment is completed, any future treatment and financials will be discussed with the receptionist.


If you have dental insurance we will help in every way we can to file your claim and maximize your insurance reimbursement for covered procedures. Please bring your dental insurance card to your appointment. We submit claims to all insurance companies. Medicaid is also accepted. We will provide you with an estimate of your patient portion based on information you provide to us. Every insurance plan provides different levels of benefits based on the coverage your employer has provided. We will ask for your estimated patient portion at the time of the appointment. For your convenience, we accept cash, check, debit card, or major credit card (Visa, MasterCard, Discover Card).
